Topic Chosen: Hourly Rounding’s
Per our hospital policy, Rounds is a standardized interaction with the patient that
includes the 8 components listed in the Hourly Rounding Procedure. Primary Registered Nurse
(RN) or licensed vocational nurse (LVN) assigned to each patient is responsible for ensuring hourly rounds are made according to policy. Nursing personnel will round every hour from
0600-2200 and every two hours from 0000-0600 and more frequently if the patient’s condition requires it. (Dignity Health Clinical House-wide Manual)
The 8 components includes
1.
Introduce self, title and remind patient of hourly round, inform patient that your are there to do rounds, and inform patient the next time you will do rounds
2.
Perform scheduled task for that hour (i.e. medication administration, dressing changes) to increase efficiency.
3.
Address “3 P’s, Pain- assess pain level per policy and intervene as needed,
Potty-inquire if patient needs to be toileted and intervene as needed, Positionreposition patient to prevent skin breakdown and promote comfort as needed.
4.
Address additional comfort needs = ensure that patient is comfortable-fluff pillows, straighten sheets, and add remove blankets and pillows, adjust lightings, etc.
5.
Conduct environmental and safety assessment of the patient room- ensure that all needed patient item are within reach-call light, phone, tv controls, Kleenex, garbage can, water pitcher etc, tidy bedside table and be sure it is within reach,

ensure bed wheels are locked, ensure that patient area is clean and neat with all unneeded items put away or thrown away
6.
Prior to leaving the room use key words to determine additional patients needs not addressed. “Is there anything else I can do for you while I am here?” or similar key word.
