Moving Toward Preventative Education in the Diabetic Patient
Chronic illnesses affect patients in all age groups in all of the ethnical and socioeconomic groups within the United States (Cohen & Cesta, 2005). The treatment and management of a chronic illness is a life-long task for the patient, the caregiver, and healthcare provider. The patient and caregiver need to understand the illness, how to manage the disease at home, but most importantly how to prevent further complications of the illness. Nurse case managers influence the disease process through the sharing of their knowledge, providing community based resources, and the relationships that are formed with the patient (Cohen & Cesta, 2005).
