Nursing Home Case Study
Case Presentation William is a single, seventy- year-old Caucasian male, who resides in a nursing home. He has no children and his only close living family member is his sister. William was admitted to the nursing home three months ago, after having suffered a stroke and subsequent left side weakness in his leg and arm. Currently, William is undergoing physical therapy at the nursing home to improve his left side mobility. William was referred to the Nursing Facility Transition program by his social worker in the nursing home, after stating he did not want to return to his sister’s home. This program is designed to help individuals in nursing homes transition back into the community. Supports provided by this program include but …show more content…
William was not feeling well on Thanksgiving and was told by his brother-in-law he needed to wait to go to the hospital because, “he wasn’t going to ruin Thanksgiving.” William continued to get worse throughout the day and suffered a stroke the next morning. An ambulance was called, and William was taken to the hospital. My role with William is to help him locate an affordable, appropriate living situation and to educate William on the types of housing available. William and I will be working together to fill out housing applications, discuss with housing options their availability, pricing and requirements for residency. Also, I will be working with the Nursing Facility Program supports coordinator to provide me with William’s physical and mental capabilities based on her assessment. At that time, she will inform me of her housing recommendations. At the request of William, I will also contact his sister if further documentation is needed for housing.

During the phone call I introduced myself, what company I worked for, ask him if he would like my help in locating permanent housing and set up an appointment to meet in person. The next day I met William in his private room at the nursing home. I introduced myself and showed him my work badge. William appeared to be aware of why I was there, and his body language suggested he was receptive to my being there. My first question to William was to ask what his ideal type of housing situation would be? William proceeded to tell me about his current physical limitations. William stated that due to limited use of his left arm and left leg, he feels he would need to live somewhere there is care on staff and that he has access to some type of emergency push button.
