William was not feeling well on Thanksgiving and was told by his brother-in-law he needed to wait to go to the hospital because, “he wasn’t going to ruin Thanksgiving.” William continued to get worse throughout the day and suffered a stroke the next morning. An ambulance was called, and William was taken to the hospital. My role with William is to help him locate an affordable, appropriate living situation and to educate William on the types of housing available. William and I will be working together to fill out housing applications, discuss with housing options their availability, pricing and requirements for residency. Also, I will be working with the Nursing Facility Program supports coordinator to provide me with William’s physical and mental capabilities based on her assessment. At that time, she will inform me of her housing recommendations. At the request of William, I will also contact his sister if further documentation is needed for housing.
