There are many levels involved in the patient-centered care as identified by Gertis et al (1993). Each of the identified levels identified by Gertis are bulleted and an explanation of how my company delivers that goal is highlighted.
• Respect for values, preferences and needs of patients
* Patients are appointed patient advocates that follow them through the healthcare delivery process * Patient’s have appointed nurses that follow the patient through the recovery period after surgery. They follow up 2 days after surgery, 2 weeks and two months after surgery. In addition, they have a number to call with questions/concerns.
* If patients do not wish to have surgery or are not surgery candidates, pain management centers are available within the organization.
• Coordination of care
* A nurse practitioner is selected for preoperative evaluation. They follow the patient through testing, MRI discussion with the surgeon, scheduling of surgery and document each phase of care with the use of an electronic record. * All phases of care are documented on the electronic record (except for anesthesia reports, which is scanned). This also includes all prescriptions that were written.
* If a patient was to have care at any of the facilities, a complete IT record is readily available to continue care at the next level/site.
• Information/Education
* The website is very informational and breaks down the surgery with computerized animation. In addition, the website has areas that define medical conditions of the spine in an easy to understand language.
