POMR (Problem-Oriented Medical Records) Advantages:
- It helps physicians retrieve information quickly & handle large or workloads; permit evaluation of physician's reasoning in assessing patients conditions; less reliance on physician memory so errors are reduced; or receives more efficient, continuous care.
-It provides documentation of comprehensive care by focusing on patient and their problems. Gives more information on the patient’s condition and care in chronologic order.
-Indicated the patient’s baseline condition for each shift.
-Entire healthcare team works together in identifying a master list pf patient problems and
contributes collaboratively to the plan of care.
POMR Disadvantages:
-It takes time to develop the problem list, it takes time to do necessary repetitious recording.
-Encourages documentation of both normal and abnormal findings making it difficult to separate patient from irrelevant information.
-Requires extensive charting time by the staff.
-More difficult to track trends in patient status.
SOMR (Source-Oriented Medical Record) Advantage:
-Very organized for each department to locate section for documentation and easy for adding loose papers.
SOMR Source-Oriented Medical Record) Disadvantages
-Cannot determine all the patient's problems and treatment quickly and must look in each area of the chart, which is timely.
-The primary disadvantage of the SOMR system is that it can be very time-consuming to find a back entry about a particular problem or treatment
SOAP (Subjective, Objective, Assessment and Plan) Advantages:
-Examining all of patient’s problems and good training tool for house staff.
-The major advantage of the SOAP documentation format is its widespread adoption, leading to general familiarity with the concept within the field of healthcare.
-It also emphasizes clear and well-organized documentation of findings with a natural progression from collection of relevant information to the assessment to the plan on how to proceed.
SOAP Disadvantage:
-Requires addition training and takes a lot of time to complete.
