Regular team meeting were conducted and input from all members of the multidisciplinary team were highly valued and respected.
I was able to contribute meaningful and relevant suggestions about further encouraging the patient to mobilise independently for short distances and relay continual patient evaluation and progress. My ability to participate in this highly effective team was acknowledged by both my preceptor and one of the physiotherapists. It was noted that my continual feedback on patient progress, assessments along with my implementation of any recommendations and my facilitation required to meet the specified outcomes was an integral component of working as a part of the multidisciplinary team and communicating effectively with one another. A direct result of the multidisciplinary team communicating effectively was the patient being discharged to
TCP.
