1. Identify the various and interdisciplinary members of these groups and organizations.
The various and interdisciplinary members of the following groups and organizations (New York City Health Department, Bronx Community Health Network, Inc., Ralph Lauren Center for Cancer Care and Prevention in Harlem, Boys Club and Girls Club) consist of city, state and federal employees, public officials, scientists, physicians, nurses, behavioral clinicians, social workers, school employees, hospital employees, community leaders and the general public.

[BCNH] is a not for profit organization, their mission is to improve the health of people and families in the Bronx. BCNH makes quality health care and preventive health services available to everyone in the community, even if they are uninsured (Bronx Community Health Network Inc., 2015). As well as, securing resources for programs and services and promote healthful lifestyles through outreach, education, and options for better nutrition and physical activity (Bronx Community Health Network, Inc., 2015). The organization monitors quality of care such as hypertension control, adolescent sexual activity screening, colorectal cancer screening and immunization of children > 3 years old (Bronx Community Health Network Inc., 2015). The organization achieves these outcomes to positively impact the public’s health by preventing health problems (offering early detection screenings, health education), providing regular checkups (flu shots, child immunization, and other essential health services, and help deal with chronic conditions (asthma, high blood pressure, COPD, and heart disease) (Bronx Community Health Network, Inc., 2015). Extended and weekend hours are available, to make the services to the community easier to access.     In the case of Mr. J, these were multiple issues that led to and contributed to his unexpected demise after what is usually considered a routinely performed procedure in an emergency department setting. The JCHAO (Joint Commission on Accreditation of Healthcare) defines a sentinel event as “an unexpected occurrence involving death or serious physical or psychological injury”, (Frain, Murphy, Dash, & Kassai, ∂ 1) and in the case of Mr. B, his death would be considered a sentinel event which would warrant a review by a team of interdisciplinary members of the hospital. In this particular case members of the team would include one or more ED physicians, the RN in the scenario and the LPN, a respiratory therapist, a nursing supervisor, a hospital administrator, the ED nurse manager, a hospital pharmacist, and a risk manager. More staff nurses from the ER could also be involved. A credible and successful root cause analysis will identify all of the elements that contributed to the event, an action plan will be developed to prevent the event from reoccurring and ensure that those actions are completed.
