The following pieces of information must be shown on a pay slip:-
Name
Address
National insurance number
Tax code
Deductions to pay
NET PA
Tax code
Employment number
Biii Identify TWO changes to personal information which you must report to your employer.
Two piecees of information that you MUST notify your employer they have changed are your Name/Surname and your Address.
Biv Describe the procedure to follow if you want to raise a grievance at work. You may describe this in writing or produce a flow chart or diagram
Bv Explain the agreed ways of working with yur employer in relation to the followinf areas:
1. Data protection
2. Grievance
3. Conflict management
4. Anti-discriminatory practice
5. Health & Safety
6. Confidentiality
7. Whistleblowing
Bvi Explain how your role contributes to the overall delivery of the service provider
There are many things that I do that contribute to the overall delivery of the service provider, for example I provide the best possible care I can give to the service users & families of those service users by communicating effectively & being involved in their day to day activities and personal needs I may also write their daily reports in deep detail.
I read and sign all the service users care plans to ensure I can provide the correct and safest care.

a) Following best practice within your work role
By following the best practice I am ensuring that the service users are getting the best possible care resulting in them being happy where they are, this also helps the service provider obtain a good name & trust
