This limited window of opportunity needs to be exploited as much as possible. Many of these residents are limited in what healthcare resources they can access because of they are on assistance or their mobility is limited.
Limits to access prevent people older adults from obtaining the healthcare they need. One of the patients I met was on disability and lived alone at the facility but had help from a nursing assistant and his daughter. He was on top of many of his medical needs because there was help to meet his daily life needs. He had someone to help with errands, cooking and cleaning. His daughter helped him to organize his medications and appointments.
The complex does have a social worker which can be a critical resource in making sure the residents can meet their basic needs and getting to a medical appointment. The social worker can help to arrange transportation and make the appointment for the resident. Many of the residents have chronic conditions that if left untreated can lead to exasperated that will bring them to the hospital.
