Presenting Problem
Pt is a 17 y/o African American male presenting to NNBHC with increase agitation and HI with plan. Pt communicated in school last week that he was going to kill his teacher and peers. Pt states that he didn’t mean these statements he was just thinking them out loud. Pt states that he is in special education and he was once in general education but he is having a hard time transitioning due all of his peers in his class are “special”. Pt mother report the pt is constantly bullied by kids in regular education and she assumes that is why he made the statements of wanting to kill his teacher and peers. He also made statements about getting the clowns to come to the school and kill his peers. Pt has demonstrated increase agitation over the weekend which includes increase verbal aggression towards parents and older brother. Pt made statements during a family meeting conducted stating he was going to kill his family members and how he made weapons. Pt made his video game controller into a nunchuck and made statements that he was going to electrocute his brother. He also had …show more content…
Also there is some history of neglect due to biological parent’s use of alcohol and substance abuse.
Substance Use
Pt. denies any history of substance abuse. However during his stay at Riverside Behavioral Health Center in June 2016, the psychiatrist stated a positive drug screen for Angel Dust and x2 unknown substances.
Family Composition
Pt currently resided with adoptive mother and father, and 18 year old brother. The pt struggles with communicating appropriately with the family when given simple prompts. Pt has demonstrated property destruction in the past due to defiance. Pt mother states the pt emotions are like a switch one moment he is fine and the next he is calling her derogatory terms.
