On Saturday, February 6, 2016 at approximately 0016 hours, FHEO Security Officers responded to a call in regards of a Disorderly Patient (53D) who refused to come out the restroom at the MRI area. Security Officers Brandon Rodriguez, Omar Alonso, and Steven Evans arrived to find a combative patient inside the restroom and refused to come out. MRI Tech, Noreen Dailey stated that the patient had been in the restroom for about 20 minutes, at which time when Noreen finally open the door, found the patient trying to pull up the sink off and acting in a strange manner like cleaning herself over and over again. In February 1998 12 year old Tiffany Applewhite went into anaphylactic shock and her heart stopped after a nurse gave her a shot of steroids for an eye condition in her family’s apartment. Tiffany’s mother called 911, and the city sent two medics in an ambulance. That ambulance didn’t have the advanced life support equipment that Tiffany needed, and the paramedics failed to bring oxygen or a defibrillator. Tiffany’s mother had pleaded with the paramedics to take her daughter to the nearby Montefiore Hospital. Instead, they advised her to wait for the private ambulance with advanced life support equipment to arrive. RA would not talk with Treva. RA constantly asked Treva to leave. RA’s daughter & family kept talking to the RA about getting services in the home, especially wound care. Family provided information that the RA will have a physician to come to the home to treat the RA’s bedsores, a nurse to monitor the RA’s meds, and an aide for the RA’s personal care. In report #17-067-00223 the RA left AMA from Hidden Lake Nursing Home. The concern was that his blood sugar was extremely elevated, and he left without taking his oxygen. Treva made a home visit to interview the RA. RA would only talk for a few minutes, but he kept telling Treva that he was fine, and he did not need any assistance from DHSS. Treva was able to explain to the RA that the concern was that he would go into a diabetic coma, but the RA ended the conversation by closing the door. Treva left her card at the door along with resources on diabetes. It was seemingly a normal quiet night on the day of March 12th 2011, until Virginia Rasnick attempted to phone her husband several times with no response. Sidney Rasnick, while on a work assignment in Jesup Georgia, checked into a motel on March 6th 2011. The hotel was owned by Krishna Hospitality INC. Throughout the work trip for all 6 days he was staying there, her husband had a schedule of always making several calls a day to her, and always one at night before he went to bed. So when he didn’t answer the phone that night she knew something was wrong. Her husband Sidney Rasnik, had a history of heart problems and she was growing more and more worried after calling five more times with no response. Desperate she asked one of the hotel operators that answered the phone to check on her husband. She stated that she was “very worried about her husband” and that he had been on medication. EREPORT # 19106 stated the following: Leigh Ann Mathis has 4 Children she has lost custody of all of them. She is not allowed contact with Preston. I know this because the Nurses at the Hospital told her to leave or they would call Police. She is now living with Kyle and Preston. She was seen with Kyle and Preston at Christmas. Kyle just had his leg amputated. He is no way is capable of taking care of himself much less an 18 month old. Between the hospital and Home. Kyle lost a pill bottle of 84 pain pills. It helped the patient to not be careful about what they were telling the doctor. Once the Tarasoft case was opened up to the courts the Confidentiality between patient and psychologist was over depending on the conversation. If a patient like Poddar comes in with psychological issues a psychotherapist should always take action. The Tarasoft decision is a California decision that imposes a duty on a therapist to warn the appropriate people or person when he/she becomes aware their patient may present a risk of harm to a person or persons. The decision made by the psychologist was not a good one at all because the women were in danger. The psychologist alerted campus police because he took what his patient said serious but did not inform her or the family. The Psychologist suggested Poddar should be hospitalized due to his mental state of mind. The psychologist should have breached the patient confidentiality once he knew third party was unaware of the patient conversation with him. Poddar was let go because he seemed to be mentally stable. Prior to starting the practicum, this student was required to sign a statement of confidentiality which outlined the parameters of confidentiality within the organization regarding sharing client information, accessing resources, altering documents and computer usage. Residents have 3 consent forms to sign upon admittance: one for consent for assessment and treatment which allows team members to talk to each other; one for the consent to disclose information between agencies (i.e. the MSDSI); and one to release information to various information to specific individuals, such as the client’s family or partners. If these forms are not filled out, staff are not allowed to share anything, including whether a resident is at home, even if the caller is the client’s parent or partner. Additionally, if a client threatens harm to self or to another person, there are procedures in place which guide how a staff member should react. At 4am dispatch received a call from the resident at 137 West Alexander Drive, who claimed to have heard a scream from his neighbours house. The young man answered with a smile on his face. I had noted that he seemed wide awake, as if he had been up for some time, as he invited us inside. At some point every year (or perhaps every 2 years) someone in a pharmaceutical organisation will say “It’s time to think about this year’s GMP training”. This statement will usually be received with little enthusiasm from those who hear it, but nevertheless a plan will be formed to deliver the training over the next 4 or 5 months or so. On the day of the training the majority of delegates will arrive with equally low levels of enthusiasm as the annual chore of GMP training begins. “Right – let’s make a start” the trainer will say, as the delegates think “well let’s get this over and done with” as they listen (whilst trying to stay awake) to the focus area of this year’s training – which is usually what they did so badly wrong last year!…
