To support individuals in their personal, social and health care needs as required.
As part of a team, working closely with families, friends, and advocates of the people I am supporting and other health and social care professionals to ensure people are supported to lead the lives they choose. Working within agreed guidelines, policies and procedures and following instruction as determined by senior support staff and my line manager. Adhering to the expected Social Care Codes of Practice at all times.
Main Duties and Responsibilities * To provide the necessary support to enable people to achieve their preferred lifestyles and ambitions as described in their person centred plan. This could include support with personal care, housework, finance, spiritual and cultural needs for example. * To follow guidelines to support people to make decisions. * To support people to maintain their rights, dignity, privacy and respect. * To work flexibly to meet people’s support needs, ensuring the day to day support is provided as required. Support and clarification will be provided from senior staff / line managers where a change outside of usual day to day support may be necessary. * To work with and follow procedures and guidelines as specified by senior staff and other professionals involved, as required. * To maintain, accurate, clear and concise written and numerical records, in respect of the people I support, team and the organisation. * To support people in taking medication in accordance with agreed policies and procedures. * To communicate and engage effectively with all people involved in a service users life. * To contribute to improving the quality of support provided through working with others. * To report any areas of concern to a senior member of staff on duty and to utilise the manager on call systems out of hours to seek advice and guidance when necessary. * To complete Induction Award
