Synapse Case Studies

I like that you discussed about Synapse. One of the issues that I have encountered with Synapse was Psychiatry and Community Mental Health & Addictions nursing staff are the only ones who have access to it. It would be nice to see that Emergency staff have access to Synapse when clients present to these areas for medical help. For the patients who frequent at the Emergency department at Mills Memorial Hospital for mental health reasons had a client centered care plan developed from either the community clinicians or inpatient psychiatric staff that is placed into a binder in ER department for future reference. For example, a community clinician worked with a client, who was diagnosed with paranoid schizophrenia, who frequently shows up emergency
department, the staff have a very detailed care plan that lays out what to refer to their medication, what topics to avoid, and keeping the conversations short and simple to keep the client from being violent towards the staff members. In my experience of inpatient psychiatric, every patient that was admitted to the unit had an individual care plan written up. The psychiatrist, nurses, and clients meet every second day to see if the client is improving and plan the process of getting the client discharge from the hospital. Inpatient care’s goals are having shorter, more intense hospital stays and begin case management in the community for the road of recovery.
