Background
Jeanelle is an 8 year old who, lives with her mother and younger brother. She takes part in afternoon programs and activities in the community. Janelle’s mom is employed full time outside of the home. She spends a lot of time with her auntie, grandfather, and other family members. She is interested in Girls Scouts and loves the sport of Gymnastics. Jeanelle has attended the same elementary school from first to third Grade. In the summary of the Forced Choice Interview conducted on 3/28/16, Jeanelle likes to receive praise and rewards when she works hard. She enjoys playing outside and wants to have interaction with friends. She has reported that her least favorite subject is Reading while, her most favorite subject is Social Studies. Currently, Jeanelle attends school and she has not missed any school days as noted by her mother and her teachers.
Reason for Referral
Jeanelle was placed on the school’s Tier II reading plan by her teacher to meet three goals within the second quarter of the school term. After conducting an extensive formative …show more content…
By not completing her work, it is significantly impacting her on level grades as well as portions of classroom management for the teacher. The teacher conducted her initial informal FBA and planned interventions in March 14 using an on tasks and off tasks tally mark system. There were twenty more marks for off tasks than on tasks noted. When assessed again at end of the week for benchmark, it was noted in the reading log stored in the classroom that her Curriculum Based Assessment Score dropped by twenty percent and produced mixed results in the benchmark growth from 90% to
