Whilst on clinical placement at Fremantle Hospital (FHHS), a patient admitted with delirium required intervention. I was tasked to collaborate with other members of the multidisciplinary team to help identify and agree upon health and wellbeing outcomes for the patient. I communicated effectively with the dietician on a regular basis via written and verbal communication in order to have the patients dietary plan reviewed regularly and to discuss any changes/improvements.
