2. I will incorporate ergonomics while maintaining comfort and safety for Mrs. Washington while positioning her or transferring her by making sure that I have enough people to help to ensure she won’t have to do anything past her physical ability, preparing any assistive devices if needed, perform hand hygiene before and after I come in contact with her, give her privacy, lock the wheels on the bed and raise the railing on the opposite side of the bed, and once she is moved I will ask her to determine her comfort and will reposition her if needed.
3. Before getting Mrs. Washington out of bed I will perform hand hygiene and explain to her what I will be doing before I come in contact with her. I will then ensure that the wheels are locked, I will have enough staff to help me if needed, and put the rail up on the opposite side of the bed to ensure her safety. Before I leave her I her chair, I will have her sit all the way back for the most support, lower the footplates so that she can rest her feet on them, and fasten the seatbelt to protect …show more content…
her from falling.
4. I will obtain a gait belt, remove obstacles, perform hand hygiene, explain to her what is happening and what she should be doing. I will always hold the gait belt while explaining everything step by step to her. We will move slowly and carefully in order for her to be as comfortable and as safe as possible.
5.
I will teach her how to safely use a cane by telling her to hold it with her strongest hand for maximum support. Put the tip about 6 inches in front and 6 inches to the side of her allowing her elbow to be slightly flexed. She should move her cane about 1 foot forward then move her weak leg forward while her weight is on the cane and the stronger leg. She should then move her strong leg forward in front of her cane, shifting the weight to her cane and her weak leg. There should always be at least two points on the floor at all times. To use a walker she will need to move it about 6 inches ahead following with her weak foot so that all of her weight is on her stronger leg, then she should move her strong leg while putting the weight on her arms and her weak
leg.
Module 4:
2. Total intake: Total output:
Clinical Reasoning 1:
1. I would without a doubt hate being dependent on others. I am a very independent person so that would be very hard on me. I would feel as if I were a burden on others.
2. I would probably not talk very much so I wouldn’t be noticed. I might get cranky and grump since I would feel so poorly about myself.
3. I would want for her to show me respect and to not talk to me like she feels sorry for me. Sometimes I would want her to pray with me if I asked and to make me feel like I’m not completely useless.
Clinical Reasoning 2:
1. No, I shouldn’t proceed with my plan to bathe Mr. King right now since he can hardly catch his breath. His oxygen is low and his respirations are high and shallow since he is trying to catch his breath. He needs to rest so he can properly breathe to fix his vitals back to normal before I try and bathe him again.
2. My priority nursing intervention is to have Mr. King rest until he can properly start breathing again. Once his vitals are back at a normal level, then I will proceed to bathe him. I will give him fluids to help him.
Clinical Reasoning 3:
1. I feel very nasty when I haven’t brushed my teeth. If I haven’t brushed my teeth yet and someone tries to talk to me I will either try and not face them or will try not to talk for the fear that they might smell my breath.
2. Frequent and proper hygiene is important to Mrs. Flowers because, just like everyone else, you always feel better once you brush your teeth. It is important to always brush your teeth because if you don’t then you could get a gum disease.
3. I would be careful of how far back I brushed on her. I will wear gloves to prevent spreading infection. When I remove her dentures, I will place them on a washcloth that I have placed in the sink in order to prevent damage in case I drop them. While removing them, I will use slight movements to carefully break the suction in her mouth. After I wash them, I will rinse them with running water to remove cleaner and food particles.
4. I, as a nurse am primarily responsible for oral hygiene for patients.
Clinical Reasoning 4:
1. I will give Ms. Rosenbaum adult diapers since she has intermittent incontinence. I would recommend a shower chair so that she could easily bathe herself comfortably. I would also suggest that she use a raised toilet seat so it would be easier for her to squat down to use the restroom.
