Therapeutic Communication
The medical dictionary describes a therapeutic communication as “A process in which the nurse consciously influences a client or helps the client to a better understanding through verbal or nonverbal communication”.

This communication summary will describe an event which occurred on a work placement in practise.
A reflective frame work will be used to evaluate the interaction; Gibb’s reflective model will be used within the essay.

whilst on placement with the district nurse we received a call from a patient informing us that her leg was not “quite right”, as she has problems with her eye sight she wasn’t able to give us a description on what state her leg was in. I went out and discovered when observing that this patient had leg ulceration which was on both sides of her left leg. I questioned descriptively to see how long she was feeling uncomfortable, she reported that it was over two months; this enabled me to place the event in time and helped me to gain a visual picture of the circumstances. I encouraged description of events by asking the patient how it went on noticed as she has carers visiting her twice a day to apply creams. It happened as a result in the carers having communication issues as they did not report changes on the breakdown of the patient’s skin. Whilst focusing on the wound I decided the appropriate form of intervention would be compression bandages. The patient was reluctant to have a bandage applied to her leg although I empathised with the patient by informing them what would happen if the appropriate bandage was not applied and that it was the only possible way of getting it to heal properly.

At the time I was thinking that the patient was not informing me properly and felt they were not telling me the full story of how the wound got to the stage it has.
The fact that I got information from the patient was good as I was able to see how they felt and what appropriate measures had to be taken. The bad part of the experience was

    According to Lloyd and Craig, taking a patient history is the most important aspect of patient assessment because information from the history is essential in guiding the treatment and management of the patient (p. 48). In this article, these two authors provide the reader with an easy to follow guide to professionally collect accurate patient information from the generalized public that is organized and prioritized through a systemic approach. The first step of this systemic process is preparing a professional, safe and private environment that is free from distractions so the patient feels comfortable disclosing confidential information to the nurse. “The nurse should be able to gather information in a systemic, sensitive and professional manner. Introducing yourself, using active listening, avoiding jargon, maintaining eye contact and holding an interested posture are examples of good communication skills. “It is important to use appropriate questioning techniques to ensure nothing is missed when taking a patient history.” (Lloyd & Craig, 2007).     The Chief of Vascular surgery, Dr. Blauhard, strode confidently down the central corridor of the operating theaters. Passing the open heart surgery rooms and the new hybrid theater he knew that his patient would be going to sleep in room 12, his room. The patient was lucky to have him as her surgeon. He had been vice-chairman of vascular surgery at a major university and had published extensively in his chosen field. He was viewed as a star. But he was not respected or revered or even liked by the staff at his new hospital. In fact, his behavior at the new hospital had been outrageous. He yelled. He criticized. He threw instruments. He humiliated nurses and technicians and even environmental services workers. He was insufferable to work with and for. So as he pushed through the doors into operating room 12 a sudden quiet fell and bodies tensed.     The goal of this relationship is not only to gather and give information but also is to promote healing and recovery of the patient. This is achieved through therapeutic communication which “ is the primary means through which nurse and client exchange information about health matters, plan treatment approaches, reach consensus about treatment decisions, conduct treatment activities, and evaluate clinical outcomes” (Arnold & Boggs, 2011, p. 175). Therapeutic communication has specific characteristics that make it different from social communication.     What I have learned about therapeutic communication is that if you listen and interact with the patient in a way that is not threatening to them, they are more likely going to share more information and be more open to answering your questions. What I have learned about non therapeutic communication is that your nonverbal behaviors play a big part in the patient and nurse relationship.
