The physician is now considering therapy with Benlysta / belimumab, which is an FDA-approved treatment for certain patients with refractory systemic lupus erythematosus (SLE). A quick review of the literature clearly shows an association between SLE and TTP, noting that lupus has many of the clinical features of TTP, with similar findings of microangiopathic hemolytic anemia (MAHA) to include the anti-ADAMTS13 antibody and anti-platelet antibody. Benlysta works agains the activity of soluble cytokine B-lymphocyte stimulator, helping to provide treatment for certain autoimmune disorders such as lupus. The request to use Benlysta for refractory TTP is reasonable, and while the diagnosis of TTP is not specifically associated with Benlysta in the literature, there is an association with Benlysta and lupus, with lupus and TTP, and with Benlysta in the treatment of MAHA associated with lupus. While this treatment must be considered experimental or investigational, it is absolutely medically necessary. There are no other treatment options for this patient’s life-threatening medical condition, and without doing anything such as Benlysta, he will likely die from his disease, making the possibility of adverse risks from Benlysta much less than doing nothing (as there are…

One of the most important choices we can make in our life is choosing to spend time with our family and in being happy. Randy Pausch in The Last Lecture expresses this point perfectly. Pausch chose to take his family to Disneyworld, which is a place him and his family both love and enjoy. Pausch still treated his family no differently than when he did not know he had pancreatic cancer. Throughout Pausch’s last six months he stayed completely optimistic and was never gloomy or depressed when around his family or friends. Although Pausch could have been depressed “At the time, I already had been diagnosed with pancreatic cancer, but I was optimistic.”(1) Pausch expresses that even though he is now informed that he had pancreatic cancer, he still is the same man as before and will still continue to be optimistic and happy every day of his life. If you work towards being something you will achieve it eventually and you will be able to remain optimistic in Randy Pausch’s case (37). According to The Art Of Making A Good Decision many people have trouble making the right choice due to their lack of knowing their priorities. In Randy Pausch’s case he chose the right choice and spent his last six months loving and showing his care for his family and enjoying every day he had left to the fullest. Randy Pausch in The Last Lecture shows many examples in which he chose to show his family he is happy despite his illness.…

One concern is the patient’s lack of knowledge about his health problems. Another concern is whether he can continue to live the way he has without assistance. Henry will not be able to care for his wife the way he needs to with his own health issues. Also his wife can no longer provide for Henry’s needs she used to fulfill, such as cooking for him. Henry’s comment about how he hopes his insurance will pay for his oxygen and inhalers raises concern for me. These health disparities may keep Henry from receiving proper care due to him not being able to afford proper health care. Also he still continues to smoke even with his illness. His breathing problems concern me too.…

“Being sick won’t change me, son,” he said to me the day he was diagnosed. “I know who I am. I worked every day for years to keep my family happy. I made a lot of mistakes in life but I did a lot of things right too. If I could go back, the only thing I would change would be becoming more diligent about mine and your mothers health. You see, son, you have to own your mistakes not let them own you.” There isn’t a day that goes by that I don’t think about that conversation.…

As a young child, I tended to become ill often. The doctors blamed my weak immune system on the fact that I contracted mononucleosis and was hospitalized for two months at a young age. The consensus was that over time my immune system would improve and I would outgrow any other issues. I continued to get very ill with bleeding sores constantly lining my mouth and throat throughout my elementary schooling. I noticed my condition was drastically worsening as I ended my sophomore year of highschool. I entered my junior year chronically ill. I missed over thirty percent of the year due to being so ill. I went almost all of junior year teaching myself the information my peers were able to learn in class. I became so physically tired that even getting out of bed was exhausting. I was living with such pain that my parents decided to seek the top…
