LT: Client will verbalize understanding of disease process and correlate s/s with causative factors by discharge 1.Review disease process and future expectations
2. Provide information appropriate to clients individual situation
3. Review clients drug therapy including the need to adhere to medication regimen, expected therapeutic outcome and side effects 1.Provides knowledge base for client to make informed choices
