On December 5, 2015, at approximately 1345 hours, the Yuma Sector Critical Incident Team (CIT) was notified of a vehicle collision, which involved an unmarked K-9 Border Patrol vehicle K95266 (herein referred to as FED-1) and a civilian vehicle (herein referred to as VEH-1). The collision occurred in the Yuma Border Patrol station’s area of responsibility, near the intersection of east 32nd Street and Windsor Avenue.
SCENE LOCATION:
From the Yuma Sector Headquarters, the area of the collision can be located by traveling east on 40th Street, then north on Arizona Avenue to 32nd Street. Turn right onto 32nd Street and head east for .65 miles to arrive at the scene of the collision.

The following is a summery of the non-recorded interview with L. Millmaker’s and is not verbatim. L. Millmaker states on December 5, 2015, she was driving in her lane (westbound middle lane of traffic) on 32nd Street and did not understand why the Border Patrol Agent next to her was driving so close. L. Millmaker, felt the impact of the vehicles and followed BPA J. Marshall off of the busy roadway. L. Millmaker stated she only noticed Border Patrol respond to the scene and did not recall seeing or speaking to the Yuma Police officer who took her statement.
In the Arizona Crash Report taken by Yuma Police Officer G. Garcia, the report shows L. Millmaker in violation of making an unsafe lane change and J. Marshall as making no improper actions leading to the collision (See Tab 12 Other Agency Report).
Additionally, Officer G. Garcia obtained a statement from L. Millmaker, which contradicts the interview obtained by BPA S. Brooks. In the statement received by G. Garcia, L. Millmaker states, she wanted to make a U-turn, started merging left, and did not know what happened when another vehicle hit her (See Tab 12 Other Agency
