The Final Transition Plan meeting was held on April 6, 2016. William’s individual sessions with therapist, Mr. Brian Schola have focused on his transition home, substance abuse and relapse prevention, as well as the recent behavioral and attitude concerns presented on the unit. William has actively participated in all individual sessions and has made goals to maintain his sobriety once release from the program. It is reported by Mr. Schola that during sessions, William has distorted briefs of independence, self-sufficiency, an inflated sense of self and entitlement from others and minimizes his potential for relapse. Furthermore, William has expressed to his Probation Officer, Ms. Ellis and Mr. Schola that his primary concern as not being substance-related but rather his relationship with his ex girlfriend. William has presented mixed emotions regarding his desire to break a court order to pursue contact with her once he is released. William has been reminded of the active no contact court order against him regarding his ex girlfriend and the potential consequences if he chooses to validate the order by Mr. Schola, Ms. Ellis and the Post D staff.
William has been adamant that he does not need continued counseling services or the number of drug screens suggested by probation once he is release from the program. However, Mr. Schola has recommended that William continues counseling to manage his substance abuse issues and to support his transition home. In addition, he will likely also need family sessions to focus on family dynamics and parental support of client’s abstinence. A family session with Mr. Schola is schedule to be held on April 8, 2016 with a focus on William’s transition back into the home and parental support for
