Late Entry

On 8/9/2016, CM met with the client and completed Bi-Weekly ILP Review. Client in the meeting appears quite. She was dressed appropriately for the weather. CM inquires how the client has being doing since the last Bi-Weekly Review. Client states in Spanish “igual” translated in English the same. Client goes on to say she doesn’t like the shelter food and most of the time she is hungry. CM advises the client if she doesn’t like the shelter food to go to Bread & Life for breakfast and lunch. CM provided client with Bread & Life address.

SOCIAL SUPPORT UPDATE: Client reports she completed Parenting classes but she didn’t provide copy of the certificate of complexion.

EMPLOYMENT: Client is undocumented

REVIEW ILP AND CLIENT RIGHT AND CLIENT CODE OF CONDUCTE: CM reiterates the shelter rule and regulation and for the client to adhere to the 10pm curfew

HOUSING UPATE; Client housing option is ticket assistant and family reunification. Client reported she doesn't have any money save to move out the shelter system and she is not leaving without her son.
ACS: Client didn't report any update court hearing and her weekly visitation is still suspended

WORKER ASSESS: Client continues to show signs of intellectual impairment. She has trouble remembering the names of people, she will sometimes forget important information that she would previously have recalled easily, such as appointments or conversations.
Plan of action: Client must seek alternative housing, client must seek and secure source of income, client must adhere to the shelter rule and regulation, client must accept the first suitable housing offer, client must adhere to the 10pm curfew, client must maintain her dorm clean and clutter free, and client must meet with CM every Tuesday. Client must adhere to the Judge Referral and recommendation. CM reviewed the Bi-Weekly ILP Review. Client agreed and
