In this case scenario I will be using a fictional character Mrs Thomas to help me encapsulate everything that happens during the admission process of a service user. Covering a wide spectrum of other scenarios during this process and showing how the home would deal and resolve them. Also making sure I put all the correct policies and procedures into place, alongside risk assessment, Health and Safety aspects and meeting the service user’s needs.
A needs assessment will be carried out in order to establish what help and support is needed for Mrs Thomas. The Care Manager is responsible for this assessment, but they may need to involve other organisations for example Doctors, Social Workers etc.
A needs assessment may take place in an individual’s own home, within a hospital, a GP’s surgery or even in a local Social Services office. Where possible, Mrs Thomas should take an active role in her assessment and should inform the Care Manager of all her needs. Mrs Thomas’s emotional and psychological needs should be taken into account as well as their physical needs. Mrs Thomas should be at the centre of the decision-making process that determines what care she needs from the home and how needs will be met.

The assessment may also include finding out about:
Mrs Thomas’s present living arrangements and arrangements for care.
Mrs Thomas’s personal health, disabilities and what they can and cannot do.
Mrs Thomas’s worries and concerns.
