Over the past 2 -3 months Kelly has repeatedly been recognized by provider's during my face-to-face visits, they have described her as an exceptional advocate for our members and valued collaborator to our providers. One-on-one provider visits are primarily led by the providers to address the topics they feel would be most helpful for them, which almost always leads to requests for a specific guided training or claim resolutions; so it was especially rare and inspiring to have several of them voluntarily make it a point to identify and praise one of our UHC CBCM's. I interviewed Joanna Hawkins. She works in a private organization for dialysis, called Fresenius Medical Care. It is in Birmingham, Alabama. She has been working with this agency for 12 years. The primary tasks she completes everyday are, going around to all the 24 chairs that they have patients at and tell them hello and ask them how they are doing. She will then ask them if they need anything, whether that be need for transportation, need for medications, need for help with their home environment, or need for general support.
