On June 2, 2016, while serving room restriction for another offense, Rashad made direct threats to harm staff and was observed attempting to provoke other residents to physically attack that staff member. Rashad has flat out refused staff directives in his most recent incident, which resulted in a 48 hour room restriction. On June 23, 2016, Rashad again made direct threats towards staff and became verbally aggressive towards staff when redirected. This incident is the latest in which he has made direct threats towards staff. In addition he remained defiant for several hours following his placement on room restriction and continued to ignore staff directives. This behavior is not only displayed in the CPP Unit, but he displays negative behaviors in the classroom and within a group setting; which results in disruption of services for other residents, as well as persuading others to be disruptive. Rashad does not appear to be motivated by the negative reinforcement initiated through the CJDH Behavior Management Program, or by the rewards-based incentives he can earn through the program commissary. Staff went to Mentoring Youth Children's Home 2700 1st Avenue, Columbus, Georgia 31904 on above noted date and time to pick up Cynnyia Tucker (13) for behavioral aide. Staff transported Cynnyia to Columbus Public Library to help her with her homework. After Cynnyia completed her homework, staff transported Cynnyia to Lake Bottom Park, 13th Street Columbus, Ga, where we remained for the rest of the behavioral aide visit. Staff encouraged Cynnyia to play basketball to promote a positive way to manage her anger by doing something she is interested in to relieve stress and negative emotions. CM, Ms. Williams (mother), Ray Onyesoh (BHH Health and Wellness Educator) attended meeting with Timothy at Mott Elementary School with Ms. Begonja (teacher), Toni Nicasio (Clinician) and Jasmine (Clinician). Merete is such a warm lady. We had a really wonderful time staying with her for couple of days! She is THE BEST host I've ever stayed with! The one teacher that made me the one I am today was coach green .he has been my coach for all four years he never gave up on me even when I was in my worst mind set.
