Unfortunately, there is a commonality that each treatment provider is faced with a client that only has one single issue. However, in this example, the client has more than one issue ranging from substance abuse to mental illness to homelessness. The provider will then need to determine if the client will need additional assistance with other issues that may be outside our scope of practice. For example, if the provider is working in a substance abuse treatment program the primary focus is helping the client to stop using drugs and/ or alcohol. As the client is no longer supposed to use substances or drink, the client can no longer reside in an environment where substances and/or alcohol are being used; this can become a trigger for the client and risk the client’s sobriety and recovery. This client is now homeless and will need assistance with housing. As the client continues in sobriety the treatment provider may notice that the client appears to be struggling with mental health issues that have not been previously diagnosed; this client will now need to be referred to a mental health provider for further evaluation and possible medication management. The provider has been made aware of these arising situations through communication and effective listening skills while interacting with the client in the group and individual therapy process. The provider will need to be aware of the client’s verbal and non-verbal cues as the clients’ affect may be incongruent with the current situation. The provider will also need to be aware of the client’s cultural issues as treatment may cause the client to have stigmatization within his or her family…

In this chapter, Garland focuses on the changing economic, political, and social conditions of late modernity to explain the shifts in crime control and criminal justice practices after the breakdown of the Penal-Welfare State. One major transformation of economic conditions is the shift into a consumer-based economy, where many minorities and low-wage workers began to feel massive economic instability because jobs became scarce and income inequality became more widespread. Through the introduction of the mass media and the television, society has become warier of government institutions as the media is now able to pry into their secrets. The social and cultural changes affected the criminal justice system in that society became more prone…

Simpson Trails, The State of Florida vs. George Zimmerman, the fictional Tom Robinson case from To Kill a Mockingbird, and The Case of Emmett Till are all examples of our flawed Justice system. Given the number of incarcerated inmates who suffer from some form of mental illness, there are growing concerns and questions in the medical field about treatment of the mentally ill in the prison system. When a person with a mental illness commits a crime or break the law, they are immediately taken to jail or sent off to prison instead of being evaluated and placed in a hospital or other mental health facility. “I have always wondered if the number of mentally ill inmates increased since deinstitutionalization” Since prison main focus is on the crimes inmates are incarcerated; the actual treatment needed for the mentally ill is secondary. Mentally ill prisoners on the surface may appear to be just difficult inmates depending on the degree of outward actions being displayed. For instance, a paranoid inmate may get into a fight simply because he believes he is being followed and/or stalked by other prisoners. It becomes quite clear that the solution for the treatment of the mentally ill is not “Incarceration”.…

What then is the best setting in which to provide the care? They must look at the scenario of developing acute care psychiatric units in prisons by shifting state funds to departments of corrections from departments of mental health. Many departments of corrections have agreements with state departments of mental health for providing acute care. This approach creates expenses associated with the transfer of offenders back and forth and security concerns, as well as interdepartmental conflicts and communication problems inherent in the difference between handling offenders and handling patients.Suicide is the third leading cause of death in U.S. state and federal prisons, exceeded only by natural causes and AIDS. Comprehensive suicide-prevention programs in prisons are of increasing importance to mental health professionals, correctional administrators, healthcare providers, legislators, attorneys, and others as they seek to rehabilitate offenders and avoid the multi million-dollar lawsuits that often arise from inmate…
