Essay On Integrated Case Management
1. In my elementary school, I would include nutrition and health services, integrated case management, and programs to help link the parents and community to the students. We all want students to learn and progress in every area possible; many times, student achievement and growth is hindered by factors before occurring before the students even get to school. When students go to school hungry or not feeling well, learning is most likely not the first thing on their minds. Also, I believe integrated case management is needed because often times, students and families who qualify for and need one service qualify for and need other services as well. These services are often offered by separate agencies, and case management would help to bring some congruence to the students and families and human services delivery. (Smith, Jr., 1995) Teachers often remark that parent involvement is lacking in schools. One reason found is that schools operate during work hours, and many parents are unable to leave work for appointments, especially those for whom every penny counts. Implementing programs to help link parents, and even the community, to the children’s education and overall well-being could really help the students improve …show more content…
I would base the services out of the school, as the school is the centerpiece for linking parents, students, and services. The school would not be the sole facility though, as part of integrated case management is helping to link families in need of services to the proper agencies in the community. (Smith, Jr., 1995) To determine who would be eligible for these services, I would use the information the school has already been provided by parents and government agencies. Students receiving free or reduced lunch would definitely be eligible, as would students who are receiving counseling services. These students and families would be first in line for other services because people in need of one service are often in need of others. (Walters,

We have many families who need assistance in areas not related to education. Their children come to school hungry, dirty, and unprepared. Some students even use the facilities to shower because they do not have running water. It becomes difficult to focus on education when our school has to “provide breakfasts and lunches, help families find housing and health care...the effects of untreated physical and mental illness, and the large gaps in children’s readiness that exist at entry to school” (Darling-Hammond, 2010). Sometimes it’s enough that the students made it to school, and we do our best to meet those basic…

Over the past three semesters all I have been hearing about is IDEA, 504, CHILD FIND, NCLB, and all of the other programs that are in place to make sure that children are not left on their own if they are in need of extra help inside or outside of the classroom. Each time any of these programs were mentioned I would always wonder about the process that a school would follow to qualify a student under these special services. I have never been exposed to the process, just told when a child was "special Ed" or "mainstreamed" in my internship classroom. I know that there has to be several steps involved in this process, otherwise there would be some major issues with parents and other professionals, when you tell them "I think your child needs special services." I also know that there has to be more than one persons opinion involved and, as in every aspect of school, parents must be completely involved and in control.…

Case management has become the standard method of managing health care delivery systems today. In recent decades, case management has become widespread throughout healthcare areas, professionals, and models in the United States; and it has been extended to a wide range of clients (Park &ump; Huber, 2009). The primary goal of case management is to deliver quality care to patients in the most cost effective approach by managing human and material resources. The focus of this paper is on the concept of case management and how it developed historically, the definition of case management, the components of case management, and how it relates to other nursing care delivery models.…

I am currently employed as a Caseworker in the unaccompanied youth at Leake and Watts Services. As well, I am doing my internship in the clinical department for the same agency. The population that I am currently working are children that cross the border from different parts of the world especially, Guatemala, El Salvador and Honduras. However, there has been an increment of children from other parts of the world, like Pakistan, Mexico, Ecuador, Romania and the Dominican Republic. The main reasons that have been provided by these children varies from, “I am running away from the gangs”, “I am running from the drugs”, “I am running away from the poverty”, “I am running away from the violence like war”. Others indicated that they come to reunify with their families or they…

Hence, the evidence that is most advantageous is to evaluate Schools with stringent policies and rules to remain cautious but put forth more of an effort in creating a friendlier, caring environment for the Students. Employing loving tactics with marginalized students will allow the students to conceptualize that their School Staff values their social, cultural, and intellectual strengths. Social Workers employed in school settings can offer additional assistance for adolescents and their families that may alleviate external issues heavily affecting them outside of the classroom. This may also increase Teacher motivation since Troubled Students would be more attentive in class. If Teachers, Faculty, Parents, Communities, began to transform our perception on Schools in penurious areas it would be the first step in producing a beneficial difference for the…

To promote involvement and positive relationships with families and communities I believe that an open door policy is essential to have. I would do this by talking with the parents every day at drop off and pick up time. During the morning drop off time I would be able to find out how the previous afternoon went, if I wasn't there to see it myself, and how the child slept the previous night. I could also find out whether or not the child ate breakfast. During afternoon pick up time I would be able to tell the parent how the child's day went. I would also keep daily sheets for each child that would include bathroom breaks, what and how much the child ate throughout the day, as well as any concerns or questions I might have about the child. Another way I would keep the open door policy is by sending home a weekly newsletter informing the parents of what would be going on that week in the classroom. I would encourage parents to call the center or stop by to see how things are going for the child. I know that when parents are working they, often times, wonder about the child's day. By calling or stopping by they are able to peek in on the child and get updates are how the day is going. I would also encourage parents to attend any special event that the child is apart of, such as Christmas programs. The parents would, also, be more than welcome to have lunch with the child on his or her birthday if they are available to do so. I believe that, as a teacher, it is important to be the support system for the child while he or she is at school and while the parents are away at work. If the parents are working on something, such as using manners or potty training, I would be more than willing to help them by reinforcing that task while the child is at school. It is very important for both the child and the parents to have consistency in their daily routine. I believe that by…
