Community nurses work across various areas, providing health care to people of any age. They provide nursing to assist in all areas of health needs to people anywhere in the community from community health centres, health clinics, schools and universities and client’s homes. This essay will focus on three aspects of community nursing, firstly, aged care nursing; secondly, the organisation ‘Silver Chain’ and finally, Holyoake; A drug and alcohol rehabilitation centre. What they are and how they benefit the community, to gain a better knowledge of what is meant by “community based nursing practices”.

Silver Chain is an organisation whose prime aim is to increase the community’s capacity to maximise its health and wellbeing. It was founded in 1905, and has assisted more than 39,000 people, delivered more than 1.2 hours of care and 1.5 million occasions of care (Silver Chain, June 20th 2010). With its 2,100 staff member, Silver Chain is able to maintain and regain independence back to clients from as far north as Carnarvon and as far south as Walpole. Silver Chain offers home care, palliative care, emergency care and family health care, free of charge. Home care is for people who find it difficult to complete day to day tasks without support; Silver Chain can help by assisting you to be able stay in your own home and continue to live in the community. For example, if a client has just had a hip replacement, Silver Chain can come in and install railings for support when walking, to make the healing process quick and more of an ease.
