G-Yellow Incident Report
I am writing this letter to make you aware of multiple incidences involving (insert name) that has made the work environment at G-yellow very hostile.
The first instance took place on (insert date). (Give details of the incident). A few days later, (give detail of incident/dates). I was taken aback by the numerous malicious actions of my co-worker. The situation has been very stressful and has taken a serious toll on me at work. I feel as though her negative attitude and demeanor affects patient care and milieu of the unit. I feel that the efficiency of the employees on the unit would be greatly improved if this issue was addressed.
For this reason, I would like to bring my concerns about the conduct of (co-worker name), to your attention.

    When I started my first job in an eye clinic, I was hired to work the front desk of the main office along with three other ladies.
