Capitation has a dramatic impact on provider incentives, and hence on provider behavior. Consider Figures 5.3 and 5.5 in the textbook, which depict revenues and costs to Atlanta Clinic under fee-for-service and capitation. Regardless of the payment system, total costs (TC), which are merely the sum of fixed costs (FC) and variable costs (VC), are tied directly to volume, so the greater the volume of services delivered, the greater the amount of total costs. The difference between the two figures is the total revenues line, and how profits and losses are realized. Under fee-for-service (Figure 5.3), the revenues line is upward sloping, and it starts at the origin. At zero volume, the provider receives zero revenue, but at any positive volume, the greater the volume, the higher the revenue. Under capitation (Figure 5.5), assuming a fixed number of enrollees, total revenues are fixed independently of volume, and hence the revenue line is horizontal. On each graph, breakeven occurs when total revenues equal total costs.…

For some time now, Americans have been wanting to switch to a universal health care system. A healthcare system where all Americans will have access to the proper health care that is affordable and fits their needs. Some solutions that can be implemented are replacing for-profit insurance companies, reforming the health care system, and hiring insurance companies that have slow cost growths. These are excellent solutions because there are a substantial number of Americans who do not have health insurance and desperately need it. However, we should not put a national health care system into effect because our current health care system is in a corrupt state and has to be addressed before we can move forward.…

Why is it that the United States, which is held out to be the greatest country in the world to live in by many and using many different standards, is the only Western country that does not have some form of socialized medicine? One reason is that opponents continue to refer to any universal health care program as just that, socialized medicine. The label alone is enough to prevent many people from supporting such an effort, without looking any deeper to the facts of the situation or the solutions it offers. The connotation behind the expression socialized medicine is that it is a system that belongs in a communist run country. Socialized medicine refers to a health care delivery system where the hospitals are owned by the government and the doctors and other caregivers, whether in a hospital, office or other setting, are all government employees. And while I agree that this is not the system that would be effective for this country and our problems, we do need a system of universal health care for many reasons.…
