WAIVER

As part of the on-the-job training of _Mr. Defensor I Gungon , (Student’s Name)
__3rd ____ year student of Jose Rizal University taking up BSHRM/CMGT____, (Year Level) (Course) under the College of Hospitality and Tourism Management, the undersigned hereby express agreement to the following terms and conditions:

That the signatories have allowed the student to undergo various activities related to his/her on-the-job training at the ______Super SUMOSAM Foods Inc._____ (Name of Restaurant/Hotel) including off-site catering/company events.

That the__Super SUMOSAM Foods Inc.___ and Jose Rizal University shall take (Name of Restaurant/Hotel) all the necessary measures to ensure the safety of the student to the best of its ability.

That the students as well as the signatory are aware of the risks while undergoing the on-the-job training including risks in traveling to and participating in off-site catering/company events.

That the students as well as the signatory hereby assume all risks related to the on-the-job training including those foreseen and unforeseen, known and unknown.

That the student and the signatory hereby release and forever discharge Jose
Rizal University, its officers and faculty from being liable to any accident and/or injury which may occur or may in the future develop as a result of the on-the-job training including off-site catering/company events.

That the signatories hereby declare that the terms of this waiver have been completely read, fully understood and voluntarily accepted.
