Outcome based practise is an activity or process that has a beneficial impact on the individuals life. It can be an action taken or a service delivered. Another way to describe it is to say that the output is the effort made and the outcome is the effect on the individual.
Outcome based approaches place the needs of the individual at the centre of service delivery, contrasting those designed by service providers.
