The way I establish a partnership approached regarding person centred approach is by talking to the service placement team within social services. They send me over care plans with basic information regarding the client and also give us information over the phone. I then read the care plan and get back to service placement team agree when we can start the care package.

I then speak to the family and social worker to gather up more information regarding the client before then going to meet the client.

I then visit the client and find out what they would like help with, what they find difficult and what they would like the carers to do step by step in each call. I also look to see what other services can also help the client …show more content…
such as meals on wheels to help with meals or day centre visit to help the client with social interaction.

When I am writing the person centred approach I always ensure that I concentrate on what the client can do such as washing their top half or making their own breakfast.
It also then identify what will keep their independence.

I then look at the person centred support plan to see what we can help with and achieving the outcomes such as what they can do, what the family help them with and what the carer can be involved in.

With one of the clients I support they had outcomes that were not realistic, The client wanted to go out to the library however they was on oxygen 24 hours a day, they was risks involved in this as the client could not come off the oxygen for a period of time.
3.2
The way I overcame this was found a library service that came to the client’s home so that the client could stay on her oxygen but still get a choice to what books she would like to read, I informed the social worker and also the family of what I had arranged.
3.3 The social worker and family agreed that this would be the best option and also cost effective as the library did the service free of charge. It also reduce costs in the respect of no transport had to be arrange and also a portable oxygen machine would not have to be
provided.
3.4 I then ensure that this was all included in the care plan and that the carer would arrive on time that day to ensure the client was up and dressed in time.
4.1
I had to ensure that I included everything in the care plan by supporting the client in her independent living. I did this by arranging for the library service to come into her house as this was something stated on the care plan something she liked doing, I then ensured I coordinated the care plan correctly to ensure the carers arrived on time.

I supported the carers by ensuring that they were given enough time to arrive to the calls and also that they had enough time to complete the call. I also phoned the client and the family regularly to ensure the care plan was running smoothly and completed quality reviews.
4.3
A few months later I had to change the care plan as the client was not able to read the books anymore and the library did not offer a service of talking books. I then researched and found a company that could provide talking books at a small fee; I spoke to the family that was able to pay this small fee. I also spoke to the social worker to see if I could put an hour social call in once a week in which the carer could sit with the client and read the newspaper with her. The way this was done was by cutting the lunch and tea calls out and carers making sandwiches and meals on wheels being delivered.

I monitored the care plan closely by speaking to the carers seeing how they are getting on in the call seeing how the client seems in herself.

I also spoke to the family and to get feedback from them to see if there was any other ways we could help their mum. The social worker also kept a regular monitor on the client to see how she thought she was doing.

I kept a record on the care plan on the compuer systems with all the contact I had in regards to the client so I had this to refer back to.
6.1
When I reviewed the care plan I involved the social worker, family and carers, this was to ensure that everyone that was involved in the careplan knew what was going on. Before the review I ensured that I had everything written down what I wanted to be included such as was the review due, does the client still require the same level of support, is the client ready for the review, will the family agree what’s happening.

Once the review was over, I then did an overall summary to ensure that all information that was needed was collected, I then typed up my minutes and sent to the social worker and the son for their records.

Once the review was over I then did a check list against what the client originally said in the very first review to see that goals were being met.
6.4
Once I had sent the minutes to the social worker and the son, I went to visit the client and read out the minutes of the meeting to ensure they was happy with them, I also gave the opportunity for the social worker and the son to email me if they had any problems with what I had written. Once this was all agreed we then set another review date for 6 month’s time.
6.5
This was then all documented on the clients records on the computer, in their paper file and also a copy in the care plan in the home. It was also signed by everyone that attended the review.
