Reflection On Form W-4V To The Social Security Administration
I mailed my Form W-4V to the Social Security Administration address listed below on March 14, 2017. I received my first direct deposit in my bank account on On May 31, 2017, as scheduled, but much to my surprise, no taxes had been withheld as they were supposed to be. I have to assume that the Social Security Administration did not receive the form.

Since I reside in Thailand, I would like to find a way to expedite this process, because the same thing may very well happen again if I mail it via post. Can you provide any help in getting this problem resolved?

Any help that you can provide would be most appreciated. I have attached a copy of the signed form I mailed to SS.

Thank you.

Best regards,

I have completed an Redetermination on Christian Stewart SSN 602-07-0009 with payee Barbara Beamon. I choose this claimant because he had a work history and payee had not reported his earnings. During the RZ payee provided all paystubs for Christian Stewart and stated he hides the paystubs from her. Payee has been the legal guardian and grandmother since birth for these SSI Claimants and has provided for them all their life. I see no indication, she is misusing the SSI claimant funds she pays rent, household expenses and personal needs for her grandchildren. In great detail I explained to the payee regarding the fair share of rent/food and household expenses among the entire public assistance household. In detail, l she reviewed her rent and…

There were two court cases that we were given to look at. The first court case was Darling vs. Charleston Community Hospital. According to McWay, this case was on a male football player that got injured during a game and was brought to the emergency room where they determined at the hospital that his left leg was broken and a doctor that did not have any experience with putting a cast on put the cast on wrong and then left it on for fourteen days (McWay, 2015, pg.78). I think the biggest mistake that the hospital made was allowing a doctor that did not have any experience in putting casts on put the cast on and the doctor put the cast on the leg wrong and when the cast was taken off there was dead tissue on the leg. This caused the doctor at…

FACTS: Eldridge was first awarded benefits in June 1968. He received a questionnaire in March 1972 from the state agency charged with monitoring his medical condition. He said his condition had not improved in the questionnaire and documented treatments he received and physicians who treated him. After the agency processed his paperwork, they determined that by letter, they determined his disability to have ceased in May 1972. Social Security accepted this termination and notified Eldridge that his benefits would terminate. As a result, he challenged this action and commenced the constitutional…

Within society there are many different types of people. Even within a culture there are thousands of subcultures and this shows as people progress through their daily lives in public places (Henslin 2005). Simply observing people through the course of a day in any given public place, you will see a vast variety of types of people and emotions they display. The public place that I chose to observe people in was the county District Court building.…
