Project Name: Tooth Fairy Dental Project
Project Description Summary: This project is to provide a build out of a dental office in Los Angeles. It requires as existing building with new interior infrastructure, negotiations with the Southern Central City of Los Angeles, and coordination with various building contractors, electricians, and supply vendors.
Project Manager: Phu, Delia, and Zhe
Date: June 10, 2013

Risk Identification My partners and I have made a list of areas that might cause the project delays or failure with their respective outcomes. We have listed the risk below that can prevent the project to finish on time. 1. Denial from the city for plans approval – this could cause the project to stop or require negotiation with another entity or even consideration for another location. 2. If city site permitting the process that require more than 3 days per submission for just a standard configurations. This would cause the project expand the launch date. 3. Delay in licenses agreement – a delay on the dental licenses and building permits, zoning, and occupancy permit can create a delay on the dental office from starting. This would cause the delay in building constructions and ultimately expand the launch date. 4. Infection control & OSHA – the lack of not being complaint with the State codes and regulations can cause the project on the on halt due to the safety of contractors’ workers. This would prolong the inspection process until up to city code. 5. Delay in equipment shipment schedule – would cause delay in the setup and deployment stages which would expand the launch date. 6. Lack of resources from contractors – job site will be full of equipment and materials that have yet install. As result would cause equipment shipment would put on hold and delay in deliver date. Also equipment will not able to install on time due to negotiation with equipment technician specialist 7. Lack of resources
