The patient suffered from severe metabolic disorders and required enteral feeding using both percutaneous endoscopic gastrostomy and jejunostomy. Additional standard feeding was implemented using formula milk and a soft diet. I was responsible to prepare feeds and operate a feed pump. Frequent nutritional screening (including weight monitoring, blood test analysis and clinical symptoms analysis) was required to modify portion sizes and deliver appropriate nutrition to the patient.

The whole approach is based on the Act Cycle which consists of the four main stages, including plan for improvements, implementing changes, evaluating outcomes, and making changes and identifying new improvements. In order to effectively incorporate such an approach into daily practice, it is essential to perform specific behaviours such as openness to failure, willingness to learn and share experiences, as well as having an ability to work within a team.
