Please fill out the below Special Accommodation Request Form to the best of your abilities and in as much detail as possible. Remember that your Special Accommodation Request Form along with your medical documentation and any subsequent documents must be submitted together. As requests are treated on a case-by-case basis, a separate Special Accommodation Request Form will need to be submitted if you wish to take another test.
