The definition of team-based care was adapted and described as “the provision of health services to individuals, families, and/or their communities by at least two health providers who work collaboratively with patients and their caregivers-to the extent preferred by each patient- to accomplish shared goals within and across settings to achieve coordinated, high-quality care” (Mitchell et al., 2012). Although health care professionals have different credential, duties and responsibilities, everyone should be coordinated and knows how to communicate among each other. Lack of proper communication especially pertinent to patient health information can lead to various adverse effect such as unnecessary waste and cost, medical errors and possibly death.

One example of lack of communication happened last week. One of the patient got admitted due to a fall that happened at his employment. Due to lack of communication between the primary care nurse, worker’s compensation claim adjuster and discharge planner, patient almost had an insurance claim denied. Patient was diagnosed properly, but the discharge planner mis-informed the insurance claim adjuster with the wrong diagnosis. Fortunately, both the resource nurse and the current nurse were very thorough and stickler with the patient diagnosis and well-knowledge with the patient’s medical condition. The result, patient became well, discharge to home, and insurance claim adjuster properly took care of patient’s health
