Yommala had called the Supervisor for assistance as the patient was emotional during her counseling session. The patient wanted to seek medical attention at the ER and reports of no SI/HI. According to the patient and Yommala, this patient is currently in a DC situation with her ex-boyfriend, whom continue to harass the patient the clinic’s premise. The patient was advise that the clinic would contact 911 if the harassment should continue and for the patient to also contact the police department in her local town as well. During the session, the patient changed her mind about going to the ER but wanted to see mental health provider immediately rather than waiting about two weeks. It was recommended for Yommala to assist the patient with seeking a sooner appointment, like today to see her therapist if applicable. After this encounter, Yommala updated the Senior Counselor about other problematic issues with the patient, referring to her DCF case and stresses in her personal life. D-Client arrived on time for his scheduled appt. with this writer. The client was alert and engaging throughout the session. During the course of the one hour session, he was aware about completing his first initial tx plan, at which he was open to discuss and complete. The client reported that he participated at HOI prior – 15 years ago for heroin. He relapses in 2009 due to his liver transplant surgery in December of 2008. When asked by this writer about happen during his relapse. According to the client, he relapses on RX medication-oxycodone. He’s been abusing his Rx medication for 6 years. This writer asked the client about how his relapse affects his livelihood. According to the client, he affected him greatly financially whereas he spent his entire $150,000 inheritance on RX medication on the street. The client further mentioned that he spend $5,000 a week on daily Rx medication of 30 capsules of oxycodone. The client expressed his disappointment in himself because he was doing well, but since his surgery, at which the client recognized as a blessing, caused his addiction to pain killers. The client continues to say, “I cannot blame anyone, but me.” The client had a desire to open up his own Bar/Restaurant…
Antwone was referral due to anger and violence incidents, especially the incident with an officer. Antwone was submitted in a medical center of US navy for a psychiatric assessment because he assaulted an officer by siping Him and beaten him and was pleaded guilty of a third grade crime. So, he was mandated to attend for therapy for about 45 days.Antwone had history of repeated violent behavior patterns and lack of self control to in moments of anger. So, Antwone's excuse for this particular incident was that the senior officer had to be beaten because of racial remarks against him. At the end of the psychiatric sessions, Antwone would be given a relevant letter of recommendation to his senior officials to prove he improved his anger management…

According to the patient, she's clean for a month, at which this writer provided a positive feedback to the patient. Then the conversation shifted about the patient's father health. Patient reported, her father has a history of heart attacks, he's 74 years old, and the doctors placed a new stem to the heart. Therapist diverted from plans to discuss respecting authority figures. Therapist informed individual that she was told by teacher that individual was very disrespectful due to individual talked back, left his seat without permission, and did not comply with requests given by his teacher. Individual states other students in class was talking also and did not get in trouble. Therapist and individual discussed and practiced responsibility and decision-making skills to avoid talking back and disrespecting authority figures.
