Witness Statement
In order to complete this section, your VLA will need to speak to your Manager to confirm the following points:
How well you minimise risk before moving and positioning individuals?
How often you prepare, move and position individuals?
How well you prepare, move and position individuals?
This will be recorded on the DVR.
Observation
As part of your Apprenticeship your VLA will observe you moving and positioning individuals in accordance with their plan of care.
