There is a nurse on duty 24 hours a day. There are conferences held regularly that includes the patient and their family, the doctor, therapist, nutritionist and social worker. This meeting with everyone involved help show the progress made by the patient. The information gathered will assist in the discharge plans. The amount of care received is substantially different in a rehabilitation center. The patient no longer needs as much care as in the acute hospital setting. Therapy is provided less than three hours a day, and three to five days a week. Nursing services may not be available 24 hours a day, but there is rehabilitation nurses. The stay at this facility is generally longer than an acute stay. As a caregiver you help patients maintain and regain health, manage disease and symptoms, and attain a maximal level function and independence through the healing process.
