This care study is about a service user with Cerebral Palsy and Dysphasia. This assignment will give a brief description of her background history, her condition and care needs. In order to maintain the confidentiality of the service user, I will refer to her as Daisy Chain and any personal information will be changed accordingly and not be used in conjunction with anything else other than this study. I obtained consent from the service user, her family and the adult day centre where she attends. Together we all arrived and agreed with the name Daisy Chain to use in place of her real name, as once again to maintain her confidentiality. (Anderson, 2010) Anderson (2010, p.53) wrote that “Consent covers more than simply gaining permission to carry out a specific procedure, typically a surgical intervention. It also involves willingness on the patient’s part to cooperate in what is proposed and in this context the level of disclosure and the way information is provided may differ”. This care study will use two of the Chapelhow et al (2005) enablers to help me reflect on and discuss care. These are communication and assessment. The links between these two enablers together with their related documentation will also be discussed.…

For that reason the remainder of this assignment will look at interprofessional working and its context within the critical incident. Day (2006) defines Interprofessional working as individuals from a profession working together for the benefit of those in their care, although they state that this does not describe the complexity and levels of the term, Miers and Pollard (2009) states that interprofessional working differs from multiprofessional working because multiprofessionals are health care workers who do not necessarily work together . The Nurse and Midwifery Council (2008) and General Medical Council (2006) state that nurses and doctors should work effectively as part of a multidisciplinary team and The Department of Health in 2007 launched the Creating an Interprofessional Workforce Programme which is designed to support integrated holistic care for patients and their carer’s. Interprofessional working has been proved to improve patient care, enhance patient safety, improve job satisfaction, streamline services and improve patient outcomes (Day 2006) and is necessary because health care has become more complex and patients now require specialised care from a variety of health care professionals with a range of skills and experience (Sargeant et al 2008). Interprofessional teams vary in…
Whilst working on my assignment, I read various journals on interprofessional collaboration (ICP), it did help me realize how important interprofessional collaboration is to the health care system. According to Alberto & Herth (2009), there is a need for faculties to incorporate interprofessional collaboration in the doctorate of nursing practice education. As a doctorate nurse practitioner it’s imperative to collaborate with other health care professionals, knowing its positive effect on patient’s safety and their health care outcomes. Most health care professional in the health care system are not trained to work as part of members in an integrated team (Johnson, 2011).…

There are skills that are essential to health/care setting that you don’t have. You can develop them through trainings. As professionals, you have to make sure that you have these skills because it will help you to understand the service user more. For example, when you’ve had training in non-verbal communication, you will understand the non-verbal messages of the service users. It will help you to overcome barriers in communication.…
Core Competencies of IPE Increased demand for care of diverse patients and cultural needs of patients with complex health care needs has necessitated increased collaboration among health care professionals, and respect for the contribution of other disciplines to provide patient centered care ((Ward, Schiller, & Goodman, 2014). To address this need, core competencies for IPE were developed by the Interprofessional Education Collaborative (IPEC) committee consisting of six health care disciplines. The purpose of the core competencies were to ensure that health care disciplines include the skills and knowledge of interprofessional education in their school curriculum. Another purpose of the core competencies are to guide educators, faculty and health care providers as they educate students to provide patient-centered care to the public and the community at large. (Interprofessional Education Collaborative Expert Panel, 2011).…
In the health care field there are a wide range of professions that all have there own important purpose. With such a huge field it is hard to know everything and that is why these different professions exist. The problem with so many professions is that they don’t always communicate efficiently. This lack of communication is where interprofessional care comes in. Interprofessional care is when people from different professions work together and communicate efficiently for the greater common good of the patient. A great aspect of interprofessional care is that it allows people from different disciplines to work together and learn from one another. Interprofessionalism allows people to come together, but that is not its only goal.…
Health and social care professionals need good communication skills to develop positive relationships and share information with people using services. They also need to be able to communicate well with people’s families and/or carers and their own colleagues and other professionals.…
As a health worker it’s your job to provide care for your service users, and too work as a team with your other colleagues to make sure this is applied. By doing this you need to have good communication skills. So that if you needed to call for help they would understand your request. Or if you needed to speak to a family member or a friend of one of the service users about something important you would be able to tell them in a professional way and they would understand you. You need to be able to communicate with everyone you work with so that vital information can be passed around, because if one person doesn’t know then someone’s needs won’t be met.…
Social responsibility, communication, and teamwork are imperative skills in the nursing field and without these skills numerous mistakes and misunderstanding would arise, which will affect the patient care directly and good quality of care will not be catered. Moreover, being assertive, empathic and inclusive knowledgeable with medical terminology and procedures in regards to the patient care would be desirable qualities for a nurse. As I will come across variety of people with different personalities, cultures and spiritual belief on a day to day basis within the nursing sector, I need to ensure that the methods and means of communication have applied adequately to the other party. Furthermore, continuous quality improvement would not be…
In almost any capacity the demands of life can be both hectic and challenging. In light of this, the processes by which life is lived, shared and nurtured must involve organization, effective communication, and collaboration. This simple concept is true of life on almost any level but it is especially true when concerning healthcare. As such, it is important to look at the effectiveness of communication and collaboration within the health care system. This consideration is important because effective communication and collaboration within the health care system has a direct correlation and obvious potential impact on the quality of care provided to the patient. The College of Nurses of Ontario defines interprofessional collaboration as “working…
Professionalism can be defined and perceived in many different ways especially when discussing professionalism within different careers. I believe that professionalism in nursing is the demonstration of proper communication, knowledge, and skill while supporting a team working atmosphere. Proper communication is not only being able to discuss treatment and issues with a patient in a mature manner, but also being able to work with a team of staff members in order to achieve a professional work environment. Communication is vastly important among healthcare workers because miscommunication or poor communication skills could harm a patient physically or mentally. According to The Journal of Trauma Nursing, “Seventy percent of preventable medical errors are a result of poor communication between providers, and more than half of all deaths in the US malpractice lawsuits are attributed to poor teamwork”…
Effective communication is the foundation for any health care setting. Communication is essential in these environments as is helps classify issues, provides emotional support and also establishes relationships. It is important for employees in health care setting’s to be equipped with social skills in order to identify wide range of emotional needs. Wilbur Schramm theory is ‘to understand human communication process, one must understand how people relate to each other’. For example a care worker needs to be able to adapt their social skills to be able to communicate with different patients and identifying their client’s needs.…
The chapters were very interesting and are filled with many things that can be applied to life. Chapter four (4) for example, has an important point about how important setting goals are. Setting goals like, getting a higher G.P.A. along with proper time management will work to my benefit and will improve myself confidence when they are realized or even when the results of having such goals take impact in my life. The chapters also highlight the fact of surrounding yourself with positive motivation of people; that is if my goal is to get a higher G.P.A. then I should associate with people that have the same or very similar goals in terms of self improvement rather than people that don’t care much or at all about their G.P.A.. Chapter seven showed why it is important to be a lifelong learner, this is important in the field that I am pursuing, Computer Science, in which things are introduced and changed within a nights notice.…
