Multi Agency Working | PC | KS | Scope | Multi-agency working is of utmost importance as to ensure that the young person is able to gain access to numerous agencies that are in place to provide them with help, information and support to manage their lives. The agencies I have regular integration with in my job role include social workers, probation officers, LAC nurses and other health professionals, members of the YOT team, CAHMS workers, teachers, advocates and the …show more content…
police.Each of these bodies provides specialist support to the young person to ensure that any problems encountered can be dealt with professionally, this provides an invaluable support framework for the young person that caters for all their needs whilst in care.
Information is shared between these organisations and collated to draw up a care plan which is designed to meet the young person’s individual needs. This information is also used when creating a young person’s risk assessment so when they initially come to us we can have adequate staffing and support for them. Multi-agency and integrated working facilitates a child centred or young person centred approach.In my job role it is important to have consistent communication with the relevant organisations to ensure that the young person is benefiting from all services available. Referrals between these agencies is required by legislation and also company policies and procedures to ensure we are providing a service which is relevant to
Every Child Matters, working together for children ,young people and families(2008), Common Assessment Framework (CAF), Better support for families (2007) and all other relevant legislations. If for instance a young person in my care was a victim of bullying within the home then I would inform my line manager who would in turn inform the CPO to investigate all allegations thoroughly and without prejudice. If this information was disclosed to me then it would be my responsibility to record this on an official disclosure form and pass it on to my manager.Each agencies primary objective is to ensure that the young person’s needs are being met to enable them to benefit from all services that are available, which in turn will promote productive and positive outcomes for the young person.Common barriers to integrated working may include lack of understanding of roles or responsibilities, an example of which may be a staff member assuming that the young person’s key worker was responsible for ensuring a young person attended a YOT meeting or medical appointment. This may arise through inadequate communication from team to team via handovers or not using the communication book effectively. | | | |
Another barrier may be that the young person has been placed a long distance away from their family home and this will impact by them not having regular visits from their social worker, this in turn may leave the young person feeling that they have just been dumped in a home and quite often will lead to feelings of resentment and anger. If this is the case the young person may see their social worker as somebody who doesn’t have their best interests at heart and will refuse to engage with them in LAC reviews and other important meetings.In these instances it is important for me to show empathy to the young person whilst also trying to re- establish a positive relationship between both parties. Also it’s important for me to ensure consistent and accurate communication to all work colleagues and outside organisations in regards to the care and welfare of the young person I’m working with.On the 12/03/12 I attended a PEP meeting at held at Orchard Farm for a young person I was working with. The meeting was attended by the young person’s social worker, a representative from connexions, our unit manager, a young person’s advocate and the young person’s parents. This meeting is an example of multi agency working and its aim is to ensure that the young person’s educational needs are being met. Other points discussed were the young person’s academic progress, targets for GCSE’s and also where the young person will be schooled in the future as he was currently being taught on site. A future meeting was planned for a date after the young person had completed a CCATS assessment as this would then allow a decision to be made on the best educational facility for him. | | | |
