In an effort to improve patient treatment outcomes, patient engagement and operational efficiency, I propose that we perform and analyze the following two-fold interdepartmental tests of change. In order to meet the requirements established by Meaningful Use, Stage 2 and facilitate clinical workflow efficiency and decrease expenses, the first part of the project will be to evaluate utilization of a web-based registration and patient portal system. The second facet of the project will involve studying the adoption of a “coordination of care” model. Currently we are limited to providing traditional GI procedures and services; we are seeking to offer our patients a more holistic approach to digestive care by providing access to a care coordinator and nutritional and fitness expertise. For our pilot program we will hire a ‘coordinator of care’ and offer the coordinated treatment plan to all of our patients. The Care Coordinator will act as a direct contact for patient concerns, and as the liaison between the patient, our providers and providers of ancillary services. We hope this model will improve outcomes, produce a more informed and satisfying patient experience, as well as create an opportunity for new revenue. The objective of this project is to improve the overall patient experience at West Gastroenterology Medical Group. In order to adapt to changes in healthcare, we must test the effectiveness of organizational modifications that may make sustainable, resilient improvements possible.
Automating registration will eliminate two bottlenecks in the check-in process; patients manually filling out forms and assistants entering the data into our EHR. The new process will effectively increase the supply of clinic time and eliminate the waste of supplies and the waste of time, caused by redundant steps. The processes design will also allow us to mitigate errors and omissions on the demographic forms, most importantly medication information.
