1. Demonstrate how to use a person-centred thinking tool in relation to own life to identify what is working and not working
Please see attached forms
2. Describe what other person-centred thinking tools would be useful in own life
Sorting what is and is not working would be very helpful to eliminate all the pointless things that are being done and saving time, decision making agreements would be helpful to decide how decisions would be made and by whom. These would be extra tools to use that would help the individual progress towards their goals
3. Evaluate which person-centred thinking tools or person-centred planning styles could be used to think more about own community connections
Communication charts could be used to see whom the client is talking too frequently and whom they are friends with locally, the relationship circle to see who they are close to. The decision making log, this could show what they are doing such as deciding to go out or what they are deciding to spend their money on. This could all help to see what the person does most and sees most therefore showing whom and what they like to do or don’t like to do.
4. Evaluate which person-centred thinking tools or person-centred styles could be used to think more about own future aspirations.
This could be found by using the what they like or dislike tool to see what they enjoy doing , good day bad day to see what they have been doing to make it a good or bad day this could all point to indicators that would show what it is this person enjoys doing and could aim at doing more of the help them get better at it.
