To provide an appropriate outcome you need to include your initial assessment findings to see if the original problem is resolved. To do this you will need to perform an assessment before the intervention and then after the intervention. For example, if you assessed that you patient was in pain, you would want to provide pain medication. You would then follow up and assess them again to see if that pain is gone; if the pain is gone then you have an effective patient outcome.…

The more detailed and comprehensive an assessment is, the better understanding we have of our patient’s and the plan of care that we will follow to ensure they are taken care of. After reading this article, I have a deeper insight into understanding the need for a structure when performing a health history. The detailed descriptions that were provided will enable one to use the specific examples when questioning a patient, ones on which I plan on implementing in my practice. I found this article very well written and explained thoroughly, as it is a great representation of a well-completed history. In my daily practice as a nurse, I follow a specific format for completing a patient history and assessment; it very closely resembles this model. I find that when initiating a patient’s history, I begin with asking all pertinent questions in relation to presenting problems, and all historical information. I then follow with a hands-on assessment, I listen to breath sounds and heart rhythms while asking questions related to those particular body system. Listen for intestinal sounds when asking questions about dietary habits. I engage the patient in their assessment so they feel a sense of trust and willingness to cooperate in their care. I believe that more articles could be written about performing a…

The space assigned to the wound care group is half of D106 classroom. The physical work area will be restricted to the table arrangement in the front of the classroom. The therapist's teaching table will be parallel to the front of the room in front of the white board to create a focal point for the participants' attention. Perpendicular to the teaching table, the therapist should also place two long tables together along the horizontal edge and make sure five chairs are surrounding the table (see diagram 1). This set-up allows all participants to see and hear the therapist demonstrating how to perform the task. One packet of materials will be placed in front of each participant, so that all patients will have easy access to materials and not have to organize them, as…

Due to the enormous growth of the health care industry itself and the many changes of how information is delivered, it is more important now than ever that nurses have acquired information literacy. “Information Literacy is defined as the ability to recognize when information is needed and have the ability to locate, evaluate, and use effectively the needed information” (Cherry & Jacob, 2011, p. 306). Nurses need the ability to recognize when information is needed because they need to know what questions they need to ask their patients to be able to clearly assess their patient’s conditions. They also need to assess outside information to be able to establish the best evidence based practice to be used in their patient’s treatment. Information can be obtained from the patient themselves or evidence based practice journals, computer data bases, and facility protocols. “To be able to achieve any of these competencies nurses need to identify where relevant information can be found” (Glasper, 2011, p. 188). It is also very important to consider the level of evidence of your source of information. Once the nurse has gathered information, one must be able to understand what the information means. Anyone can read the words on a lab report, but nurses must be able to know what lab values mean. They must also be able to know when values…
