Patient's Case Summary
The patient is a 29 year old male who presented to the ED with SI and multiple plans to harm himself. Patient reports having problems at home with his wife and feeling hopeless and worthless. The patient reports looking for a high enough place to jump off to kill himself. Per documentation the patient was in his ED room and pulled a pocket knife out and tried to cut his left wrist.

During the time of assessment the patient was sleep and was awaken. The patient was non aggressive and willing to undergo TACT assessment. The patient reports that yesterday his wife and him got into an argument over a male she has been seeing. The patient reports that he became irritable because his wife was going to take his children Trick or Treating with this individual. The patient reports that after the argument he left and went to his grandfathers house. Further, the patient expresses during this time he drunk 12 Red-bulls and snorted 2-4 grams of cocaine in a attempt to get his heart to burst and end his life 10/30/15. Patient reports this did not work, this morning he went back to his home because it is his daughters birthday and nobody was there. The patient reports leaving his home he went to the store to buy some alcohol and go to the park. Patient reports that he has been to DayMark, however was unsuccessful in the services he would have liked.

Due to an increase in depressive symptoms, suicidal ideation with a plan, history of attempts, harm to self, patient does meet criteria for IVC and inpatient hospitalization. TACT consulted with Dr. Haviland and it was recommended to refer for inpatient hospitalization for safety and stabilization. TACT assisted the ED doctor in completing IVC paperwork. TACT will search for appropriate

    On Thursday December 31, 2015 at approximately 1949 hours, FHEO Security Officers were dispatched to room #503 Beds 1 and 2 for two Disorderly Male Baker Act Patients who were acting up and causing problems for their assigned Psych Sitter, including one patient from Bed 1 who kept trying to leave the room.
