RBERT N MCDONALD: Case Study

MR. ROBERT N MCDONALD has been seen in our office on April 29, 2016 and May 6, 2016. The patient has a seriouse infection to his left leg. The patient requires IV antibiotic to manage the condtion. My concern is, the infection may become systemic and the patient may becom septic. It is my advise for the patient to be evaluated with infectionuse deases MD and to develope further treatment plans to minimize the malevent outcome.

Severe peripheral vascular disease, status post deep ulcer on left toe. Rule out thrombolysis. The patient was admitted to a regular floor. Condition is serious. 2. ALLERGY TO PENICILLIN, which puts patient into anaphylactic shock. 3. Continue with home medications. DETAILS OF PRESENT ILLNESS: Mr. Kinsey is an 87-year-old white gentleman with history of (1) Chronic atrial fibrillation, on Coumadin. (2) Chronic deafness, bilateral. (3) Severe peripheral vascular disease. (4) Status post left endarterectomy done in November of last year. (5) Chronic renal failure. (6) Status post right great toe amputation secondary to peripheral vascular disease. (7) Status post cataract surgery. (8) CHF; class 1. (9) Incontinence of stool. The patient was seen by Gary Shelton, DPM, on 18 February with a history of 3 weeks’ ulcer on the toe. Podiatrist referred the patient to Tomas Burgos, Vascular Surgery, who suggested that the patient should have an arteriogram. The patient also saw Dr. Hirsch today because of the ulcer on his toe. Dr. Hirsch’s decision was that the patient should be admitted to the hospital for incision and drainage of the ulcer as well as IV antibiotics. As an outpatient, the patient had x-ray at the provider DPM's office. Also, the patient had bone scan done on 18 February, which was reported as increased uptake in the mid aspect of the left 1st metatarsal, right metatarsal head, and the 3rd phalanx, cannot rule out osteomyelitis.     My patient during my RN shadowing experience had cellulitis in their left lower extremity with an ulceration below the knee, thus creating a portal of entry for the bacteria. Presence of black tissue around ulcer with purulent drainage. Redness to left lower extremity with warm scaly skin, tender, 3+ edema with diffuse borders. Patient showed signs of fatigue, general malaise, and muscle aches and pains. He had joint stiffness in affected extremity due to swelling of the tissue. Blistering was present on affected leg with leakage of serous exudate. His white blood cell counts were elevated to compensate for the bacterial infection. C-reactive proteins were also elevated due to soft tissue inflammation. Compression socks were applied to affected extremity, immobilized the leg and elevated on pillows while laying supine. He was treated with Amoxicillin, which interferes with cell wall replication of susceptible organisms by binding to the bacterial cell wall. Also prescribed Percocet, which inhibits ascending pain pathways in CNS, increases pain threshold and alters pain perception. Area of erythema was marked with a pen and circumference of leg was measured to observe if the cellulitis is spreading or if it is being resolved with treatment. Dressing was applied to ulceration below knee to maintain aseptic technique. Vital signs and lower extremity monitored and assessed…

    When McDoland’s venture’s out globally they have to use important standards of marketing. The two most important things that I have noticed that got changed with McDoalds strategy is their product presentation and their advertising strategies. In some countries they sold the same product as in the USA. Countries that did this was Bulgaria, Denmark, and Oman. The reason why they had similar products is due to the similar taste that the USA and these other countries share. In countries like the Philippines, Taiwan, and Thailand the food is much different then in the USA. The reason being is that Culturally they have a different dietary habits then us. So in those countries they ended up doing offerings like crispy chicken, rice, and spaghetti which is important so they can cater to regions needs and taste especially when it comes to something as important as food! Also it is important to note that one of McDonald’s great models is to standardize their popular items here and make it taste and looks the same whether its in Singapore, Spain or South Africa website. McDonald’s also has to be adaptive when it comes to building their franchises in other countries in order to gain success. Therefore the concept of “think global, act local” has been clearly adopted by McDonald’s and it is all over there international websites. One last thing to add about product development is the Indian website. In that country they had to rethink the way that they presented and processed their food according to the Indians tastes, value system, lifestyle, language, and perception. Mostly in all countries everyone loves their traditional hamburgers which are prepared from beef and pork. Since Indians don’t eat pork, McDonalds had to rethink their strategy which was to introduce chicken, lamb, and fish to satisfy the Indian Market.
