When I visited her I first noticed that she had taken off her grown and was only in her incontinent brief and had the sheet over her. I quickly performing her ADLs and found out that she had no street clothes to change into and so I decided to put clean hospital gown on her. I had to perform her oral care prior to her breakfast arrival tray at 0800hr and I still had not visited my other patient to check in on him. I did not wanted her daughter to arrive early and see her mom in this state not be decent and ready to go. I also had to perform her vitals and administer her medication before she leaves. I felt overwhelmed and was sweating, just by moving in and out of her room and performing her care and taking her vitals. I wanted to make sure that her vital was not above or below her norms prior to medication administration. In her past medical history she was diagnosis with dementia and so each steps of her care I had to reminded her of what I was doing and prompted her to what I need her to do that she has the capabilities of doing. It was now after 0800hr and I was just setting her up for breakfast and I still have yet to administer her medication. I went to get her medication and did my Medication Administration Record (MAR) check on the computer against her medication and got my clinical instructor to do complete a second check. At this time it was now 0830hr and when I returned to her room with the medication to do my finial medication check at the bedside prior to administrating the medication, she was eating having her breakfast and her daughter was now there. I introduce myself as a student nurse from Centennial College and I was assigned to her mother and will be administering her medication. I did my bedside checks then administer her medication. I asked her daughter if she had brought any street clothing for her mother and she did. I told her that I would return to change her mother’s clothing in a few minutes. I had
