Reflective Account On Taking Care
I was able to manage my assignments effectively. I prioritized each patient that I should be taking care by their needs. I always started by listening to shift report and looked for a patient that has impairment with Airway, breathing and circulation. I took care of those patients who are unstable right away. For example, I took all my patients vital signs and reported the valued to the nurse. I informed her that one of my patient blood pressure was elevated and we gave him blood pressure medications. I retook his vital signs in one hours and it was within normal limit. I saw my remaining patients as by need basis. Most of my patients were stable and they asked for water and snacks occasionally. I provided them with what they have requested as long as they did not have restriction. …show more content…
Next, my biggest challenged was when I walked into a patient room and introduced my self and told them that I would be coming again to fulfill their needs when I could have accomplished their needs right there. It felt heart rendering, but I have to prioritize their needs between stable and unstable and time management. One of things that I did which made me felt like I made a difference was my assignments and patient teachings. I found out that my patient has myopia and tinnitus which no one has asked him before and it was not on his medical record. I informed the nurse about and recorded it. I took another patient who initials was C J. I got to witness a thoracentesis procedure and 1500ml of fluid was removed from both her lungs. With the removal of fluid, she was able to breath normal and felt better. These are my two nursing diagnoses and interventions which I am going to explain

Patient is a 61-year-old white male admitted through the ER with on December 10 with recurrent right pneumothoraxes. Patient is known to have COPD with emphysema and has multiple admissions for problems concerning this. At the time of initial evaluation, a small caliber chest tube was inserted in the anterior axillary line, which improved the patient’s respiratory distress but did not completely resolve the pneumothorax. I was called to the ICU to place a second small caliber chest tube in the posterior axillary line below this. This further improved the patient’s pulmonary status with his saturation improving from 76& to 89%. Since admission he has felt better but complained of pain at the chest tube insertion site. He has continued to leak out through the pleur-evac under water seal, and beginning yesterday he developed subcutaneous emphysema, which has gotten progressively worse. Earlier today he began having increased respiratory difficulty again, with his saturation dropping to approximately 80 % despite oxygen per nasal cannula. Chest x-ray today showed a worsening of the right lower lobe loculated pneumothorax, and on examination today he is not only leaking air through the pleur-evac system but also around the two chest tubes.…

Patients with a small PSP without breathlessness may be discharged with early outpatient review along with clear written advice to return in the event of worsening breathlessness. If you are a staff nurse, each morning you check the assignment board to see which patients you 'll care for that day, gather your report sheets, and enter the conference room. You 'll listen to reports focusing on the important issues and jotting down notes to help you remember the items later in the day. Chances are that you have a system for taking report that even involves writing the important issues in certain locations on your report sheet for easy retrieval later. As you leave the conference room, you 're thinking about meds due in the next hour and organizing your medication cart. You make rounds on your patients and do assessments next. Whether you use a written list or a mental one, you 've already determined what important events need to be taken care of during your shift. Who is going to surgery and needs the preoperative checklist completed? Who is going home and needs discharge teaching and confirmation of transportation? This affected my physical safety because I didn't jump ahead of anyone or rush and I took my time so that I would be more safe and listened to the instructions despite that it was repeated many times. I also waited for those who weren't as confident and let them take their time and waited for them to be done so that everyone was safe and I knew that they would be safe.…

Reflective practice is something we all carry out every single day, probably without even realising it. We use reflection in many different situations and under many different circumstances, as a way of improving our own skills and abilities for different things, as individuals. Cowan proposed that learners are reflecting in an educational way, “When they analyse or evaluate one or more personal experiences, and attempt to generalise from that thinking” (1999: 18). Reflective practice allows us to look at something we have carried out and allows us to see whether or not we handled the situation properly or whether we would handle it differently if we were giving the opportunity to do it again. By doing this reflecting enables you to see how you have improved in certain areas and pinpoint areas which may need some improvement. It’s basically a way of seeing what you have learnt and perhaps shedding light on what could be done in the future. Moon’s theory runs parallel to this as he says that reflective practice is “a set of abilities and skills, to indicate the taking of a critical stance, an orientation to problem solving or state of mind” (1999:63). Suggesting that reflective practice is directly linked with the thinking you do surrounding something you…

Good time management and organization skills are the key ingredients for success in nursing. A good tip is to always keep a clipboard with a list of the patients and information about them. Information would include how they use the bathroom, if they need to be turned or fed, or equipment they may have. A successful nurse assistant should be able to see the unit’s needs as a whole instead of only focusing on your present assignment. Prioritizing the patients will help one stay organized throughout the shift. Taking fifteen minutes at the beginning of the shift to organize the work assigned can certainly make a difference between a smooth shift and a hectic one.…

“Take away love, and our earth is a tomb”, a popular quote from an English poet and playwright Robert Browning, that best describes the play Romeo and Juliet by William Shakespeare. Star-crossed lovers Romeo and Juliet felt that they should be together and they did not care what it took. They were willing to give even their lives. Their love ended in death. The play Romeo and Juliet showed that the ruling passion of true love surpasses the three other kinds of love shown in the play; physical, courtly and family love.…

Evolution is referred to the process of change in the genetic make-up of a species over time. It’s used to explain the emergence of new species. Evolutionary theory holds that existing species of plants animals have over millions of years from simple organism. Although the theory of evolution is usually associated with Charles Darwin, idea that modern plants and animals could change was posited by a number of scholars prior to the mid 1800’s, but none of the earlier theorist suggested a unified theory that explained evolution. (Scupin, 48) Charles Darwin and Alfred Wallace both developed natural selection by combining their studies, but there were major differences in their life-experiences.…
